Your natural testosterone levels may decline for a variety of medical reasons. Common causes may include: obesity and weight gain, pre-diabetes and diabetes, obstructive sleep apnea, depression and medications for depression, chronic medical conditions/pain conditions, medications like narcotics and corticosteroids-like prednisone, disease of the heart/lung, kidney and liver. If you are experiencing symptoms, such as tiredness, fatigue or poor sex drive and erectile dysfunction, you may qualify for testosterone replacement therapy. Men who have used anabolic steroids and may be suffering with low testosterone secondary to a condition known as Anabolic Steroid Induced Hypogonadism – ASIH.
Many fine physicians do not feel comfortable prescribing and managing men on testosterone. My private medical practice is the right place for you. Most of my new patients are men already on TRT that are unsatisfied with their current medical supervision. TRT needs to be managed by an expert physician only.
There may be potential cardiovascular dangers associated with TRT (see my video on TRT and heart disease with Cardiologist Dr. Brett Nowlan), in addition to complications associated with hair loss, mood instability, acne, gynecomastia-enlargement of breast tissue, prostate disease(s) (see video), increased viscosity of blood, blood clots, elevations of blood pressure, ankle edema and other serious medical issues. Further, many physicians simply prescribe gels and creams as treatment, underestimating the potential for spreading testosterone to partners and children. Testosterone is a controlled drug and studies have shown that it can be abused and lead to dependency. Always discuss possible risks with an expert physician before beginning any treatment.
Before starting any treatment, I will evaluate your medical history and perform a physical exam that focuses on your symptoms. This includes:
Laboratory studies to measure your testosterone levels. I will also select specific studies to assess your cardiovascular health, prostate health and other, "reversible" conditions that may have led to a low testosterone state.
Medical conditions that could complicate a testosterone regimen, like diagnosed or poorly treated diabetes, heart disease, hypertension, hypercholesterolemia, blood (hematological) disorders , including a personal or family history of deep venous thrombosis DVT or pulmonary embolism PE, obstructive sleep apnea, psychiatric issues like depression and anxiety in addition to prostate disease will be reviewed in detail during my intensive evaluation.
We will discuss the risks and benefits of TRT before beginning any treatment. Not all men with low or low normal testosterone require treatment and I will discuss this very important aspect of care with you in detail. This is one of the most important aspects of my medical practice that sets me apart from other experts in testosterone replacement therapy. You will have open access to me, at any time, with personalized treatment that you would not get from a group of health care providers. After beginning TRT, most of my patients receive laboratory testing every four to six months, others more frequently if we are monitoring a medical issue closely. The frequency, details of laboratory testing and clinical care is based on Evidence-Based-Medical studies, but most importantly, applied on a man-per-man basis.
